We ALWAYS offer free consultations if you would like to talk with Dr. Kuzmak. He will always take the time to listen to your concerns and off his professional opinion.
Payment is due at time of service.
We have weekday and even SATURDAYS appointment times available in June, July, and August.

Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.
Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements. We ABSOLUTELY require payment at time of service. We will not treat you without completely informing you of fees and having your written agreement. Being fair and having no surprises are paramount.
Dental Insurance
If you have a PPO dental insurance plan, you can come to us as a new patient.
Please contact our office for Insurance details. We participate with MANY dental insurances including AETNA and CIGNA and many more. We also participate with Connection Dental which is a this umbrella insurance, there are many plans covered.
Copays for in-network insurance based patients is to be paid at time of visit. Out-of-network patients will need to pay for the complete fee also at time of service and the benefit that your insurance will cover will be mailed directly to you.

Care First Blue Cross Blue Shield:
We are not contracted with Blue Cross / Blue Shield / CareFirst. Payment is due at times of service. We will submit claims to CareFirst and they will send their own payment via direct mail to you directly.
Delta Dental:
We have many patients that choose us as their Delta Dental provider. However, we are not contracted as an in-network provider with Delta Dental. What this means is that Delta will pay you directly for our services. They reimburse between 60-100% of our fees. Many Delta Dental plans pay us 100% of preventative (exams, cleanings, and X rays), and 80% of everything else.
You will be responsible to pay us at time of service, and Delta will pay you via mailed check directly shortly after.
